Why might a child use a different name?
A child may want a different first name or surname for identity, family or personal reasons. Listen to what matters to them. Autism creates no separate deed poll rule; a school can discuss respectful daily use of a preferred name while you check how to update legal records. The school register, EHCP and NHS record may follow different processes.

Who must agree to a child's legal name change?
Check everyone who holds parental responsibility (PR), including anyone named in a relevant court order. Separation does not automatically remove PR. GOV.UK says High Court enrolment needs everyone's agreement or a court order; its school guidance says schools should obtain independent written evidence before changing a child's surname.
If there is disagreement or a special guardianship order, get independent family law advice before buying a deed poll. GOV.UK says a 16- or 17-year-old may make their own unenrolled deed poll; the rules for younger children differ.

What are the deed poll options for a child?
For a child under 16, GOV.UK describes an unenrolled deed poll prepared by a specialist agency or solicitor, or High Court enrolment. Its free make-your-own unenrolled wording is for people 16 or over. Ask the receiving organisation what it accepts before choosing a route.

How do I update the EHCP, school and GP records?
School: Ask the headteacher, office and SENCO how to update the admissions register, daily-use name and related systems. Ask what consent and evidence they need.
EHCP in England: Write to the local authority SEND caseworker about the plan and linked case file. Check the child's support and placement details remain accurate. A name change alone is not a new EHC needs assessment.
NHS: Ask the GP practice for its secure name-update process. NHS England says a patient who changes name by deed poll should give the practice the appropriate document.
Other records: If your child receives Disability Living Allowance (DLA), GOV.UK says to report their new name to the DLA helpline. A child passport application also needs a name-change document, proof of the new name in use and written consent from everyone with PR. Keep a checklist of requests and confirmations.

What if a record holder will not update a name?
Ask which evidence or consent is missing and request the record holder's written policy. A school may distinguish a daily-use name from the legal name on its register. While you resolve this, ask staff how they will address your child respectfully. Keep correspondence, use the organisation's correction process if needed, and avoid buying another document until you know why the first was refused.

Does my child need an enrolled deed poll for school?
Not always. Ask the school what proof and parental consent it requires. High Court enrolment creates a public record; an agency or solicitor can prepare an unenrolled child deed poll.
Will a name change stop EHCP support?
A name change alone does not rewrite EHCP provision. Ask the SEND caseworker and school to confirm their records identify the same child and raise any missing support separately.
Can I use GOV.UK's free deed poll wording for my child?
GOV.UK says its make-your-own unenrolled wording is for people aged 16 or over. Its under-16 guidance describes an agency, solicitor or High Court route.
Do I need to tell DLA and HM Passport Office?
Tell the DLA helpline if your child receives DLA. HM Passport Office lists further evidence, including parental consent and proof the new name is in use, for a child passport.
